It makes sense to use actual thrust and to ignore the lever position. If your actual forward thrust is in takeoff range and you are not configured for takeoff, you need an alert, no matter where your thrust lever is.

Apropos of REV flag ...

Quote1.1.158. EICAS thrust reference mode indication now blanks when a REV flag appears.

1.1.159. EICAS thrust reference mode indication now blanks when reverse lever 2+3 or 1+4 are pulled (theory based on real single engine reverse ground test and KLM video).

...

1.1.165. Modification 1.1.159 removed and modification 1.1.158 restored (source: BA sim).

Looking at the latest test, it's probably correct that the status of just one engine is sufficient, and not of a pair (2+3 or 1+4). However, it looks like it's not the REV flag but the reverser lever which blanks the green EICAS thrust reference mode indication:
When pulling the reverser levers from forward idle, the green "TO" indication promptly disappears, a fraction of a second before the REV flags appear.
When the reverser lever is slammed to forward idle from maximum reverse thrust, the green "TO" indication re-appears promptly, while the amber REV flags are still displayed.

Just cosmetics anyway ...
