Maybe you have found this information already, bust just in case:
I assume you are looking for the ICAO Document 7910.

The following source (not Wiki it self but from Wiki ) is dated 2006, so not really up-to-date, but yot fully outdated either:

You may as well purchase the latest version of Doc. 7910:
http://www.icao.int/icao/en/sales/cat_2009_2010_en.pdf
and search for "Doc. 7910".

Yes, I have the r4cdlocationindicatorsbystate.pdf already. But that's an airport list only. I have the full airport names already in my database, they don't need to be looked up.

"Doc. 7910" is an airport list also. I'm looking for 2-letter ICAO country codes.

The 5-letter waypoint database is a map killer (see white dots): ca. 135,000 points (enroute and terminal). I let the names and triangle symbols appear only at high zoom settings -- and the map still gets lots of overlaps.
