Sadly too many flight crew just "pitch for the sky" and don't try and feel the aircraft through the maneuver.

If you look at the FDR plots from these events, most are usually the result of over-control through the lack of aircraft response to pitch input, resulting in a hurried addition of elevator followed by the aircraft response.
